Aging of the face is inevitable and as we get older, it becomes more difficult to retain the youthful, healthy glow. Age, sun exposure, gravity and genetics all contribute to the aging face. The skin begins to appear drier and thinner. Collagen, the protein that binds the skin together, begins to break down, and with time the skin of the face and neck loosen. Fine lines around the eyes and mouth, become wrinkles and gradually deeper folds develop. The jaw line softens into jowls and beneath the chin, excess skin starts to sag giving the appearance of an extra chin or a “turkey neck”.
While you can’t stop the aging process, you can “Turn Back the Hands of Time”. Rhytidectomy, or facelift as it is commonly called, has become the third most desired cosmetic surgical procedure. A facelift cannot stop the aging process, but it can restore a more youthful appearance and help you look your best.

There is no ideal in a facelift. Skin type, ethnic background, degree of skin elasticity, individual healing and basic bone structure are factors that are considered during the consultation. We offer three different facelift procedures:

Doctors, Silver and DeJoseph spend at least an hour with each new patient during consultation to review not only the procedure, but the patient’s goals and desires. “Video Imaging” is one of the most helpful, communicative tools as it allows the patient to try on the facelift. As we discuss how the procedure is performed, you actually see your own image restored to a youthful appearance. It allows you to try on the facelift and it gives you the opportunity to address any other areas of concern.
“You never go from the consultation to the operating room,” states Dr. William Silver. “After deciding to schedule the facelift, I bring the patient back in for a pre-operative appointment. This gives me an opportunity to re-evaluate their medical history and review individual risks, especially medical conditions such as high blood pressure, a tendency to scar, smoking, and any deficiency in blood clotting. I think of the preoperative appointment as a teaching experience for both myself and the patient. A traditional facelift was our only option years ago, but with all the new advancements in medicine and technology, face-lifting procedures have become more individualized giving patients more options for facial rejuvenation. A traditional full facelift (rhytidectomy) will not stop the aging process, but it can set back your clock by improving the most visible signs of aging. Removing excess fat, tightening underlying muscles, and re-draping the skin of your face and neck will take you back 10 to 20 years. A facelift can be done alone, or in conjunction with other procedures such as a forehead lift, eyelid surgery, or nose reshaping.

The amount of undermining naturally varies from patient to patient. Many patients think you go from the consultation to the operating room, but this is not the case at Premier Image Cosmetic & Laser Surgery. This is done to review the patient’s history, medications, EKG and blood work. “It’s what we call an “H & P” (History and Physical)”, says Dr. DeJoseph. “Safety is top of mind, and it gives us an opportunity to sit down and review the “Video Imaging” that was done during the initial consultation. It allows the patient to ask more questions about the procedure and address any concerns.”
The morning of surgery and during the preoperative preparation, Doctors Silver and DeJoseph mark the face with a marking pen, the exact design of the planned incisions varying with each patient according to the amount of skin excision, direction of pull, and so forth.
“I’ve reviewed the patient’s photographs the evening prior to the surgery for planning, but the morning of surgery, during the preoperative preparation, I mark the patient while he/she is sitting up,” says Dr. Silver
. “The patient will be supine or lying down during the surgery; however my exact design will still be in place.”
“The goal here is to restore a very natural look,” says Dr. DeJoseph
. “The direction of pull depends on the deformity that is to be corrected in the individual patient. Planning of the surgery begins at the consultation and continues through the pre-operative appointment, to the night before the procedure, and finally, the morning of the surgery.”
The traditional full facelifts are usually performed under general anesthesia and MD anesthesia will be with you during your entire procedure. Typically there is very little, if any, discomfort after surgery; however medications are prescribed by your surgeon to ensure comfort is maintained. Some numbness of the skin is quite normal; it will disappear in a few weeks.
Be sure to follow your surgeon’s instructions after surgery. He will give more specific guidelines for gradually resuming normal activities. Please avoid strenuous activity for at least two weeks. Do not use curling irons, or hair dryers (set on cool is okay)and avoid steam baths and saunas for at least 2 months.
