Aging of the face is inevitable and as we get older, it becomes more difficult to retain the youthful, healthy glow.  Age, sun exposure, gravity and genetics all contribute to the aging face.   The skin begins to appear drier and thinner.  Collagen, the protein that binds the skin together, begins to break down, and with time the skin of the face and neck loosen.  Fine lines around the eyes and mouth, become wrinkles and gradually deeper folds develop.  The jaw line softens into jowls and beneath the chin, excess skin starts to sag giving the appearance of an extra chin or a turkey neck

While you can’t stop the aging process, you can “Turn Back the Hands of Time”.  Rhytidectomy, or facelift as it is commonly called, has become the third most desired cosmetic surgical procedure.   A facelift cannot stop the aging process, but it can restore a more youthful appearance and help you look your best.  

Doctors William E. There is no ideal in a facelift.  Skin type, ethnic background, degree of skin elasticity, individual healing and basic bone structure are factors that are considered during the consultation.  We offer three different facelift procedures:

  • the Traditional Full Facelift,
  • the “QT Mini Facelift”, featured on Fox 5 News, as the best low down time alternative to the traditional full facelift,
  • and the Mid-Facelift. 

Before we discuss these three facelift options it is valuable to understand how important the consultation can be in deciding which procedure is ideal for your needs and goals.

Doctors, Silver and DeJoseph spend at least an hour with each new patient during consultation to review not only the procedure, but the patient’s goals and desires. “Video Imaging” is one of the most helpful, communicative tools as it allows the patient to try on the facelift.  As we discuss how the procedure is performed, you actually see your own image restored to a youthful appearance.  It allows you to try on the facelift and it gives you the opportunity to address any other areas of concern. 

“You never go from the consultation to the operating room,” states Dr. William Silver.  “After deciding to schedule the facelift, I bring the patient back in for a pre-operative appointment.  This gives me an opportunity to re-evaluate their medical history and review individual risks, especially medical conditions such as high blood pressure, a tendency to scar, smoking, and any deficiency in blood clotting.  I think of the preoperative appointment as a teaching experience for both myself and the patient.   The “video imager” is once again, a useful tool as the patient and I review the photographs and discuss expectations, concerns and recovery time.”

Traditional Full Facelift

A traditional facelift was our only option years ago, but with all the new advancements in medicine and technology, face-lifting procedures have become more individualized giving patients more options for facial rejuvenation.  Here we discuss the traditional full facelift, however consultation with one of Premier’s Board Certified Facial Plastic Surgeons is paramount in deciding if a full facelift is optimal for you. 

A traditional full facelift (rhytidectomy) will not stop the aging process, but it can set back your clock by improving the most visible signs of aging.  Removing excess fat, tightening underlying muscles, and re-draping the skin of your face and neck will take you back 10 to 20 years. A facelift can be done alone, or in conjunction with other procedures such as a forehead lift, eyelid surgery, or nose reshaping.

Silver and DeJoseph begin the incision in the area of the temple hair, above the ear.  This provides more lift to the cheek area restoring what gravity has pulled down and gives a more natural appearance rather than a pulled or stretched look.  The incision continues down in a natural line in front of the ear (or just inside the cartilage at the front of the ear), and then continues around the earlobe, circling behind, but remaining in the crease of the ear, and finally sweeps back into the hairline. 

The tiny suture material combined with the experienced hands and advanced techniques of Doctors Silver and provide for a completely camouflaged suture line.  “Much of the suturing is done inside,” says Dr. Silver. “This provides for longevity of the facelift and allows me to individualize the lift according to the patient’s needs.”

The amount of undermining naturally varies from patient to patient.  Many patients think you go from the consultation to the operating room, but this is not the case at Premier Image Cosmetic & Laser Surgery.  Even though the consultation was in depth and careful evaluation and diagnosis were provided, the patient returns for a second evaluation or what we call the preoperative appointment.  This is done to review the patient’s history, medications, EKG and blood work.  “It’s what we call an “H & P” (History and Physical)”, says Dr. DeJoseph.  “Safety is top of mind, and it gives us an opportunity to sit down and review the “Video Imaging” that was done during the initial consultation.  It allows the patient to ask more questions about the procedure and address any concerns.”  

Traditional Full Facelift , Dr. Silver & Dr. DeJosephThe morning of surgery and during the preoperative preparation, Doctors Silver and DeJoseph mark the face with a marking pen, the exact design of the planned incisions varying with each patient according to the amount of skin excision, direction of pull, and so forth. 

“I’ve reviewed the patient’s photographs the evening prior to the surgery for planning, but the morning of surgery, during the preoperative preparation, I mark the patient while he/she is sitting up,” says Dr. Silver.  “The patient will be supine or lying down during the surgery; however my exact design will still be in place.” 

“The goal here is to restore a very natural look,” says Dr. DeJoseph.  “The direction of pull depends on the deformity that is to be corrected in the individual patient.  Planning of the surgery begins at the consultation and continues through the pre-operative appointment, to the night before the procedure, and finally, the morning of the surgery.”

Doctors Silver and DeJoseph apply a dressing to protect the entire area where the incisions have been made.  This will be removed the morning following your facelift.

Typically there is very little, if any, discomfort after surgery; however medications are prescribed by your surgeon to ensure comfort is maintained. Some numbness of the skin is quite normal; it will disappear in a few weeks. 

Be sure to follow your surgeon’s instructions after surgery.  He will give more specific guidelines for gradually resuming normal activities.  Please avoid strenuous activity for at least two weeks.  Do not use curling irons, or hair dryers (set on cool is okay)and avoid steam baths and saunas for at least 2 months.  For helpful tips to prepare for any cosmetic procedure, visit our “Surgery Education” page or click here:

Mid-Facelift/Cheek Lift

Endoscopic procedures are also referred to as Minimal-Incision surgery as it is performed using a tiny fiberoptic lens inserted through half-inch incisions hidden in the hairline. The lens is attached to a camera connected to a television, which allows Doctors Silver and DeJoseph to have complete visibility throughout the procedure.   Dr. William E. The Mid-Face Lift or Cheek Lift procedure is particularly advantageous for those patients with early signs of aging and those who have sagging cheeks, deeper nasal labial lines, thinner lips and down-turned corners of the lips.  The Mid-Face Lift does not correct the neck or brow areas.  You can simulate the results of a Mid-Facelift by placing a finger next to the corner of your mouth and gently lift the skin up and out or by taking your thumbs and lifting the cheek area up and out.

cheek lift atlanta mid faceliftA "browlift" or forehead lift is used to correct a drooping brow line (eyebrow area) reducing the horizontal lines and furrows (worry lines) that often make a person appear angry, sad or tired.  The procedure creates a refreshed, youthful appearance to the forehead area above the eyes.

When performing a forehead lift procedure, muscles and tissue areas that droop or cause creases are removed or corrected to smooth the skin on the forehead to reduce frown lines and raise the eyebrows.  There are two surgical methods, and during your initial consultation the procedure that works best for you will be recommended.  The first is the most conventional method where a surgical incision is hidden just above the hairline.  Browlifts are also performed using an endoscope which enables the procedure to be performed with a minimal number of incisions.  While both methods can attain a satisfactory browlift, the method that is right for you can be determined during a consultation with one of our physicians.

Chin Enhancement

Chin enhancement can be carried out with a chin implant, which can be placed through a small incision or through fat transplantation or injection of long-lasting, semi-permanent “fillers”, eliminating the need for a formal surgical procedure.
